Patient Pattern and ADVault Align to Improve Quality Advance Care Planning For High-Risk Individuals
Frailty Risk Score used by over 2,000,000 people bundles with digital platform to drive more person-centered care. First client instance goes live in January 2021.
BUFFALO, N.Y. and DALLAS, TX – Patient Pattern, the health industry leader in frailty risk stratification solutions, and ADVault, Inc., creator of the world’s leading digital advance care planning platform MyDirectives® and MyDirectives MOBILETM, announced today an integrated solution for their enterprise clients to improve healthcare outcomes. A collaboration process that began in early 2019, the two companies turned doctors’ frequent calls for advance care plans during the COVID-19 crisis into an opportunity. The first joint client is expected to go live in January 2021.
The new bundled technology offering will allow clients to identify at-risk patients and help them develop advance care plans tailored to their individual needs. This will improve provider, patient and family morale and satisfaction by aligning care with the individual’s clearly stated goals and reduce low-value care.

Frailty involves decreased function across multiple body systems. As frailty increases, so does the risk of negative outcomes. For over 30 years, frailty scores have been validated in the United States, China and Australia and are even mandated in Canada and the United Kingdom. Patient Pattern’s proprietary Frailty Risk Score has been optimized and automated for over 2 million patients in the United States across care settings, with an 89% confidence in predictive value.
Patient Pattern’s clients will have seamless access to ADVault’s suite of enterprise-facing tools, such as its facilitated advance care planning web-based engine. These modules help doctors and nurses engage in meaningful conversations with patients and allow users to update or create advance care planning documents that can be securely stored and shared in ADVault’s global repository, the ADVault ExchangeTM. Documents in the Exchange can be shared, updated and accessed 24/7 in any medical record, anywhere in the world in accordance with healthcare data content and transport standards.
